How Did Lucy MacLean Lose Her Finger?

Lucy MacLean loses her finger to the Ghoul. In season 1, episode 4, "The Ghouls", he's tying her up when she bites one of his fingers off, so he cuts off one of hers with a knife in return. Later that episode, a robot surgeon at an organ harvesters' clinic sews a replacement onto her hand, a spare finger from a drawer full of them. (Fallout, "The Ghouls")
How Lucy ended up as the Ghoul's prisoner#
Lucy is only a few days out of the vault. She's chasing the severed head of Dr. Siggi Wilzig, which she needs to trade for her kidnapped father, and so is half the wasteland. When a gulper swallows the head in a flooded ruin, the Ghoul ties her up as bait to lure it back out. He tells her he isn't torturing her. "I'm using you as bait." When that fails, he marches her off into the desert as his prisoner. (Fallout, "The Head")
Her vault manners don't survive the trip. She lectures him about the Golden Rule, and he answers that the wasteland has its own. (Fallout, "The Head")
The finger#
In "The Ghouls", Lucy fights back while the Ghoul is tying her up and bites off one of his fingers. He cuts off one of hers with a knife in return. He then sews his finger back on. Ghouls heal so well in the show that it reattaches, while Lucy is left with a bleeding stump. (Fallout, "The Ghouls")
It's one of the season's clearest early lessons for Lucy: the people up here play by different rules, and her good manners won't protect her. A woman in Filly had already noticed the difference two episodes earlier, sizing up Lucy's clean hair, nice teeth and "all ten fingers". (Fallout, "The Target"; Fallout, "The Ghouls")
The replacement#
Out of vials and close to collapse from withdrawal, the Ghoul sells Lucy to organ harvesters in a ruined Super Duper Mart for a two-month supply. Inside she meets Snip Snip, a cheerful Mr. Handy surgeon. Its first words to her are about the injury: "I say, you've lost a finger." (Fallout, "The Ghouls")
The robot sits her on a gurney and pulls out a drawer of spare fingers, rejecting a couple as too fat or too withered before picking one. It tells her she's lucky it didn't have to use a thumb, since its finger inventory is in a sorry state, then attaches it and offers her a choice of bandages, including real cotton. Lucy is grateful right up to the moment she realises the clinic plans to harvest the rest of her. (Fallout, "The Ghouls")
She overpowers the robot and kills the harvesters. Outside, the Ghoul has collapsed, and rather than leave him to turn feral she gives him the vial he needs before carrying on alone. (Fallout, "The Ghouls")
Does the finger matter later?#
Not much. Interpretation: the show treats the swap as dark comedy rather than a plot point. Lucy carries on with a stranger's finger and nobody mentions it again in season 1 or season 2, but the scene is the first time the wasteland physically changes her, a small version of the harder person she becomes by the end of the season.
