No. As of the end of Fallout season 2, the Ghoul is alive and heading off to find his family, and Amazon has confirmed Walton Goggins is back for season 3. He isn't undead either: ghouls are living people mutated by radiation. Here is what the show has aired so far.
No, the Ghoul is not dead. At the end of Fallout season 2, Cooper Howard is alive, has just found his wife's and daughter's cryo pods empty under New Vegas, and walks away to find them. Amazon has confirmed that Walton Goggins returns as the Ghoul in season 3. He isn't undead either: in Fallout, a ghoul is a living person mutated by radiation, not a corpse. Is the Ghoul alive after season 2?#
Yes. The season 2 finale, "The Strip", ends his storyline on a beginning rather than an ending. With Mr. House guiding him, Cooper reaches the cryo bay where Barb and their daughter Janey were meant to be frozen. Their pods are empty, but Barb's holds a postcard of Colorado. For the first time in 200 years he has proof his family may be alive, and he leaves to find them. (Fallout, "The Strip")

Is the Ghoul in season 3?#
Yes. Amazon's season 3 page, updated in June 2026 when filming began in Los Angeles, lists Walton Goggins as Cooper Howard, the Ghoul, among the returning cast, alongside Ella Purnell as Lucy MacLean and Aaron Moten as Maximus. The times he nearly died#
The show likes putting him in the ground. Season 1 introduces him in a coffin, buried by an enemy, until bounty hunters dig him up to help them catch a defector. He kills two of them and takes the job himself. (Fallout, "The End")
His real weakness is chemical. Like many old ghouls he needs a drug from small inhaler vials to stop himself turning feral, and without them he weakens and collapses. He trades Lucy to organ harvesters to get more, and she still leaves him a vial rather than let him turn. (Fallout, "The Ghouls")
In the season 1 finale he takes on a squad of Brotherhood of Steel knights at Griffith Observatory and walks away from it. (Fallout, "The Beginning")
Season 2 comes closest. After he tranquillises Lucy to trade her back to her father, she punches him out of a window and he lands on a pole. A super mutant pulls him off, treats the wound and asks him to join a war against the Enclave. He says no. (Fallout, "The Wrangler"; Fallout, "The Other Player")
Ghouls aren't dead in the first place#
Part of the confusion is the look. Ghouls appear burned or rotten, often without a nose, but they are people who were mutated by radiation instead of killed by it. They can live for centuries, heal from wounds that would kill anyone else and shrug off radiation. They aren't zombies; a sane ghoul thinks and talks like anyone else. (Fallout 3; Fallout, "The End")
The danger is going feral, losing the mind and attacking anything that isn't a ghoul. That, not death, is the threat hanging over the Ghoul, and it's why the vials matter so much to him. (Fallout 3; Fallout, "The Ghouls")
Interpretation: that is also why he has outlived nearly everyone he knew. He has been alive since the day the bombs fell, and Fallout 76's Burning Springs update already has him hunting bounties in Ohio around 2105, almost two centuries before the show.
