Isa Itou is the Gestalt Elster keeps running into at S-23 Sierpinski, a girl from Rotfront looking for her lost twin, Erika. She was Ariane's school friend, and her story ends in her family's bookshop.
Isa Itou is a Gestalt, a natural-born human, who turns up at S-23 Sierpinski looking for her twin sister Erika. She introduces herself to Elster as "Isa, Isa Itou"; the medical database in Rotfront files her under her full name, Isolde. She grew up in Rotfront's Sektor C, went to school with Ariane Yeong and worked in her family's bookshop. (Signalis)
Who is Isa Itou?#
The archived medical database Elster pulls up in Rotfront has records for both twins. Isolde and Erika share a birth date and a world of birth, Vineta, and both list Rotfront's Sektor C as home. Isolde is a shop assistant; Erika is a university student. Isa's history is a list of injuries, a fractured shin, a double fracture of the right arm and a forehead laceration, and the doctor calls her "very prone to accidents". The family ran a bookshop in Sektor C. Ariane remembers seeing The King in Yellow in "the shop window of the bookstore where the twins lived" before the Protektors confiscated it, though she can't recall whether it was Erika or Isa who told her. On the Penrose-512 she hears a Vinetan accent in Elster's voice like her old friends Isa and Erika's. (Signalis)
What does Isa do at Sierpinski?#
Elster meets her in the facility library just after Isa has finished off a corrupted Eule with a knife. She tells Elster that "you don't belong here either", that something is wrong with the place, and that she has something to do too. A note she leaves in the room is addressed to her sister: she has come to look for her, "despite everything that's happened". (Signalis)
If Elster finds her again on an upper floor, Isa asks her to pass on a message if she meets her sister, then turns inward. She wonders if you can speak to someone for the last time without knowing it, and says she can't keep relying on others, "that's why she left me after all". (Signalis)
Later scenes show her standing her ground. Adler stops her outside the commander's quarters and offers to "help". In the mines he hunts her with a rifle, saying she doesn't belong there; she gets behind him, stabs him in the eye and he falls into a hole. Deep in Nowhere a caged, many-armed creature has her cornered until Elster draws it off, and Isa brings it down with Adler's rifle before collapsing. Woken with a small bottle, she hands Elster the rifle, says she doesn't want to use it again, and complains that she feels sick and the air is so thick "you could bite it". (Signalis)
The library memory#
After Adler throws Elster down the lift shaft, the game plays a memory from Isa's point of view. She wakes in an empty classroom at the Mandelbrot Polytechnische Oberschule, wondering whether Erika went home without her. In the school library she sees students beating Ariane. Isa watches and does nothing, and the text flashing on screen points to Erika as the one who stepped in. That moment seems to sit behind Isa's line about relying on others. (Signalis)
How Isa's story ends#
In the third chapter, a short scene shows Isa staring at her blackened hand in a mirror, more badly wounded than before. Elster finds her for the last time in the Itou bookshop in Rotfront. Isa says she looked everywhere, "but she's not here anymore", says she can't go on, apologises, reaches for Elster and sinks into the floor. To reach that part of the district, Elster has to enter Isa's own number from the medical database into a door. (Signalis)

What is stated and what is theory#
The game states that Isa came looking for Erika, never found her, and dissolved in the bookshop. It never says when the twins died, how, or whether Isa was ever physically at Sierpinski. The shrine is the main reason most players think both sisters were dead long before the events of the game, which would make Isa one more memory the world has been rebuilt from. That is a reading, not something any document confirms. (Signalis)
The intro cutscene shows Isa cutting her hand over a symbol drawn on the floor. Players tie this to The King in Yellow and to the ritual site in the secret ending, and some argue Isa's ritual helped bring the facility to its current state.
