Is Kerrigan Dead?
No. At the end of StarCraft II, Kerrigan doesn't die: she becomes a xel'naga, destroys Amon and comes back two years later for Raynor in human form. Here is why some players think she died, and what the game actually shows.
No, Kerrigan isn't dead. At the end of StarCraft II she ascends as a xel'naga, destroys Amon and survives. The final cinematic, two years on, shows her walking into a bar on Mar Sara in human form to collect Jim Raynor, and he leaves with her. (StarCraft II: Legacy of the Void)
What the ending shows#
In the Into the Void epilogue, Kerrigan, Raynor and Artanis follow Amon into the Void. They free Ouros, the last of the xel'naga, who tells them only a xel'naga can defeat Amon there. He gives his essence to Kerrigan. When it's done, Ouros is gone and Kerrigan has become a towering figure of golden energy. (StarCraft II: Legacy of the Void)
She leads the last assault and destroys Amon. As she does, she tells Raynor to go, and he's swallowed by white light. (StarCraft II: Legacy of the Void)
The cinematic Homecoming then jumps two years. Raynor sits alone in Joeyray's Bar on Mar Sara, where Wings of Liberty began, looking at old photographs. The door opens on golden light and Kerrigan, red-haired and human-looking, asks, "You about ready to get out of here, cowboy?" He leaves his marshal's badge on the table and goes with her. (StarCraft II: Legacy of the Void; StarCraft II: Wings of Liberty)
Why people think she died#
The word sacrifice. The epilogue's completion message talks about the Koprulu sector recovering and making the most of Kerrigan's valiant sacrifice. Interpretation: the sacrifice is what she gave up, her life as a zerg and her chance at an ordinary human one, rather than her death. The same epilogue shows her alive afterwards. (StarCraft II: Legacy of the Void)
The disappearance. The closing text says Raynor was never heard from again and that his badge was recovered from the bar. Some players take that as a hint that both of them are dead, or that the cinematic is Raynor's dream. Interpretation: the scene plays as a reunion, and the deep page on the Legacy of the Void ending reads it that way, but the game never explains how a xel'naga takes human form or where they went, so the cautious reading isn't ruled out. (StarCraft II: Legacy of the Void)
Ouros does die. The xel'naga who empowers her fades away as the last of his essence leaves him. It's easy to blur the two, but it's Ouros who dies, not Kerrigan. (StarCraft II: Legacy of the Void)
Zeratul's vision. In Wings of Liberty, Zeratul sees a future where Amon has won and Kerrigan, the one being who could have stopped him, has been killed. That future is what the whole trilogy is about avoiding, and it doesn't happen. (StarCraft II: Wings of Liberty)
Has she died before?#
Not in the story. She has been changed more than any other character in StarCraft: infested by the zerg in the original game, turned back into a human on Char at the end of Wings of Liberty, and reborn on Zerus as the Primal Queen of Blades in Heart of the Swarm. In Heart of the Swarm's missions she can fall in battle and regenerate, but that's gameplay, not story. (StarCraft II: Wings of Liberty; StarCraft II: Heart of the Swarm)
Where she is now#
Nobody knows. Nova Covert Ops, the mission packs set after the End War, follows Nova Terra and leaves Kerrigan out. The timeline in Timothy Zahn's novel StarCraft: Evolution dates the battle in the Void to 2508, the only firm date the story gives for her ascension. Since then, she hasn't appeared. (StarCraft II: Nova Covert Ops; StarCraft: Evolution)
So the honest answer: alive, a xel'naga, and last seen leaving a bar with Raynor. (StarCraft II: Legacy of the Void)

What Happened to Kerrigan After Legacy of the Void?
Kerrigan became a xel'naga, killed Amon, handed the Swarm to Zagara and later came back for Raynor in human form. Here's what Legacy of the Void's ending shows, what it leaves open and where she is in the stories that followed.

Why Did Kerrigan Become a Xel'naga?
Kerrigan became a xel'naga because it was the only way to kill Amon. In the Void, the dying xel'naga Ouros told her only one of their kind could defeat him there, and she accepted his essence partly to make up for her past. Here is the reasoning Legacy of the Void gives, and why it had to be her.

Sarah Kerrigan, the Queen of Blades
Sarah Kerrigan was a psychic terran who began as a Confederate ghost, rose to be second-in-command of the Sons of Korhal, and after Arcturus Mengsk abandoned her on Tarsonis was captured and infested by the Zerg Swarm to become the Queen of Blades.

Amon
Amon was a malevolent xel'naga who turned against the Infinite Cycle of his kind and sought to end all life and remake it in his image. He created the Overmind and the Zerg Swarm, enslaved the Tal'darim, and was finally destroyed in the Void by an ascended Sarah Kerrigan during the End War.

Artanis
Artanis was the Hierarch of the Daelaam, a renowned warrior who rose from executor during the zerg invasion of Aiur to lead the reunited protoss. He forged the Khalai and Nerazim into one people, reclaimed Aiur, and led the alliance that banished Amon from the Khala.

Jim Raynor
James Eugene Raynor was a farm boy turned Confederate marine, marshal, and rebel who helped bring down the Terran Confederacy and later fought its successor, the Terran Dominion, becoming one of the few terrans to ally long-term with the protoss.
