How Did Anakin Survive Mustafar?
Obi-Wan left Anakin Skywalker legless, armless and on fire beside a lava river on Mustafar. He lived because Palpatine sensed he was in danger and got to him in time, and because the dark side kept him going long enough to be rebuilt as Darth Vader.
Anakin survived Mustafar because Palpatine got to him in time. Obi-Wan cut off his legs and remaining arm and left him burning beside the lava, but Palpatine sensed his apprentice was in danger, flew to Mustafar and found him still alive. What Obi-Wan did to him#
The fight on Mustafar ends with Obi-Wan on higher ground and Anakin trying to jump past him. Obi-Wan cuts off both his legs and his remaining arm, the right one having already been replaced after Geonosis. Anakin slides down the black sand to the edge of the lava river. Obi-Wan tells him, "You were the Chosen One!" and Anakin screams back, "I hate you!" Then the heat sets him on fire. (Revenge of the Sith)

Who saved him#
At the same time, Palpatine, now Emperor, has just fought Yoda in the Senate. He senses that his new apprentice is in trouble and flies to Mustafar with a squad of clone troopers. They find Anakin on the bank, burned and limbless but alive, having dragged himself up from the lava with the arm he had left. Palpatine has him put in a medical capsule and taken back to Coruscant. (Revenge of the Sith)
There, droids graft on prosthetic limbs and seal his body into the armour, mask and breathing system. The first thing he hears as Vader is Palpatine's voice: "Lord Vader, can you hear me?" His first question is about Padmé. Palpatine tells him that, in his anger, he killed her. (Revenge of the Sith)
How did he stay alive long enough?#
The film never explains it in so many words, so the rest is a reading of what it shows.
Interpretation: three things carry him through. He's the most powerful Force user the Jedi had found, which the films stress from his first scene with the Jedi Council. He's full of rage at Obi-Wan, and as Anakin himself put it earlier, the Sith rely on their passion for their strength. And Palpatine wants him alive badly enough to come in person, at once. Any one of those on its own might not have been enough. (Revenge of the Sith)
What surviving cost him#
Anakin never walks free of that suit. Rogue One shows Vader in a bacta tank in his castle on Mustafar, a scarred figure out of the armour, and the castle stands on the same world where it happened. (Rogue One)
Obi-Wan Kenobi picks up the thread ten years on. Obi-Wan has lived in hiding believing his former apprentice is dead, until the Inquisitor Reva tells him Anakin survived. Vader blames him for what happened on Mustafar, tells him "I am what you made me" when they meet again, and in their last duel says he isn't Obi-Wan's failure: "You didn't kill Anakin Skywalker. I did." (Obi-Wan Kenobi, "Part II"; Obi-Wan Kenobi, "Part III"; Obi-Wan Kenobi, "Part VI")
Interpretation: that line answers the question from the other side. In Vader's mind, the man who could have died on Mustafar already had. What survived was the Sith Lord. (Obi-Wan Kenobi, "Part VI")
